Referral Forms

Licensed prescribers may fax or phone in prescriptions to any Vital Health location per your patient preference and location.

For IV Infusions

An EMR-generated prescription order with details will suffice OR download our referral form, complete and fax to us OR use a manufacturer’s PSP enrollment AND fax this form is faxed directly to the number below at Vital Health Pharmacy (please indicate our pharmacy name on the form also if sending to the PSP).If you require pre-vaccination prior to Biologic start, please send both the vaccine form and the biologic prescription simultaneously if possible. We will delay biologic start until clearance from your office is provided.

Patients Transfer In

To transfer in, all we need is a phone call or visit from you, with your permission we will transfer your records from previous pharmacies.

Fills and Refills

New prescriptions may be presented in person, or by mail. A fax or photo-image via e-mail is not sufficient, unless immediately followed by delivery or pick-up of the original paper Rx. (Faxes can only be received from MD offices and considered as originals). Refills once on file at our pharmacy, may be ordered by phone or e-mail.

We deliver at not additional cost to patients, throughout Greater Victoria and Greater Kelowna.
